* For any credit card payments, a credit card authorization form is required if the payment is not made in person. Please contact the office for the form.  A 50% deposit is required upon registration. Deposits made with check payments will require a copy of the check at the time the registration form(s) is returned via fax or email, with the original check immediately being sent via regular mail. TSG reserves the right to reschedule or cancel any course date(s) for any reason, including insufficient enrollment. In such instances, deposits will be returned or applied towards the next available training program. If you wish to cancel, reschedule, or change students, 5 days notice must be given. Please allow 5-7 business days to receive DOB approved cards and 4-6 weeks to receive OSHA cards. All DOB approved course completion cards will be mailed to the individual that made the payment, unless otherwise requested. All OSHA cards MUST be picked up in person by the student that took the course, NOT by the individual who made the payment. No refunds or credits of any payment will be made for notification of withdrawal within 3 days of the start of the class, once the class has started or for missed classes. The Safety Group Ltd. discloses that only the learner shall be entitled to access to his or her training records and/or related information.  In the event that a replacement certificate is needed, the learner must visit The Safety Group Ltd. office in person, pay the required fee, and show proof of identification before such replacement documentation is issued.
Please be advised that the SST card price is not included in the price of the course/class and will require a seperate payment.

 I attest & acknowledge all the following statements:

I am the person that registered for the class and was given secure access to the online session.

I fully understand that I must have a working knowledge of the internet and the approved electronic device I will be using to attend the online course.

I fully understand that I must have continuous access to an “approved electronic device” that has a working camera and a reliable connection to the internet.

I fully understand that I must actively participate, attend, and complete the entire course as per the established amount of time without any form of assistance from anyone else.

I fully understand that I must be able to read, write, and verbally communicate in the language the class is being instructed in.

I fully understand that the entire class will be recorded for compliance and training purposes.

I fully understand that I may not make audio or video recordings of any class activity at any time.

I fully understand that if I am found to be inactive or not physically present during any span of time that is not deemed a break by the trainer I can be removed from the class. In the event, I am removed from the class for non-participation, I understand that I will have to complete the entire class over again at my own expense.
